Hospital Discharge Support

Leaving the hospital can be a stressful time, especially when ongoing support is needed at home. At MercyBridge Care, we provide comprehensive Hospital Discharge Support to ensure a smooth, safe, and well-coordinated transition.

Our team works closely with hospitals, allied health professionals, families, and support coordinators to develop a personalised care plan that meets your specific needs — right from the moment you're discharged.
